Please choose the store you visited by typing the street name, or city.
Select the date of your visit
Did you make a purchase on this visit? [You can give only one answer.]
After completing the BAM customer survey, you’ll get a validation code that can be used for a discount on your next in-store purchase. The offer may vary, but it’s typically something like:
$5 off your next order
Or a percentage discount (like 10% off)
Check your purchase receipt for the exact reward offered at the time of your visit.
